Pacific Hospice Care has special expertise working as a
partner with nursing homes. Our skilled admission nurses
and counselors are always available to assist.
Pacific Hospice Care Physicians work with the patient's
physician and are available for pain and symptom management
consultation as well as to visit the patient in the nursing
home as needed.
Pacific Hospice Care Physicians are experts in pain and
symptom management and use innovative techniques to control
pain and manage symptoms, specially designed to meet the
unique needs of patients who will benefit from our specialized
supplemental care.

Nurses |

Pacific Hospice Care Nurses work with the nursing home
staff to establish an integrated plan of care and to
supplement their nursing care.
Pacific Hospice Care Nurses are skilled in assessing
and managing pain and symptoms.

Home Health Aides |
|
|
Pacific Hospice Care Certified Home Health Aides
help the nursing home provide additional care including
additional baths, skin care and dental hygiene and also
help with activities like playing cards, reading from a
favorite book or taking the patient for a walk.

Social Workers |

|
Often a prolonged illness can place patients and their
loved ones in a crisis situation. The Pacific Hospice
Care Social Worker is available to assist in stabilizing
families in crisis and to help with important issues and
concerns.
Pacific Hospice Care Social Workers assist patients and
families in finding community and financial resources that
may reduce financial stress, with completing government
forms and documents, with financial issues and planning
and as friendly and skilled listeners to provide emotional
support.

Spiritual Counselors |

Pacific Hospice Care Social Workers and Spiritual Counselors
help with day-to-day financial, social, spiritual, family and
emotional issues that can accompany a prolonged illness.
Pacific Hospice Care Spiritual Counselors are available to help
patients and families of all faiths work through spiritual
concerns, provide assistance with spiritual practices, prayer
and spiritual readings as requested and are available to work
with the family's clergy.

Trained Volunteers |
|
|
Pacific Hospice Care provides trained volunteers to
aid the family and patient, staying with a patient to
give family members a break, lending emotional support
and companionship to the patient and family members,
playing cards or games, listening to music together,
going for a wheelchair walk or reading from a favorite
book. Perhaps their most important task, however, is
their ability to be good listeners.
