Pacific Hospice Care patients are cared for in their own homes
or the home of a loved one or nursing home. The goal is to help
keep the patient pain-free and with loved ones nearby. A caregiver may be a life partner, relative, or friend.
They are trained to work closely with staff to help with feeding, bathing, turning,
administering medications & monitoring changes in a patient's condition,
with Pacific Hospice Care support every step of the way.
Pacific Hospice Care has established partnerships and is pleased to
provide care to residents of Skilled Nursing Facilities (SNFs), Long
Term Care (LTC) Facilities, Assisted Living Facilities (ALFs) and
Residential Care Facilities for the Elderly (RCFEs). Pacific Hospice
Care has special expertise working as a partner with nursing homes.
Our skilled admission nurses and counselors are always available to
assist. Pacific Hospice Care supplements the nursing home's care by
providing equipment, medications and supplies that the nursing home
does not ordinarily supply. The patient's physician can continue to
visit and direct care.
Pacific Hospice Care accepts Medicare and Medi-Cal as 100% coverage
for services, so there are no out-of-pocket expenses for drugs,
equipment and other high cost items. All services, visits by medical
and nursing professionals, home health aides, medications, treatments,
continuous nursing during a medical crisis, inpatient services,
supplies and equipment of Pacific Hospice Care are fully covered 24
hours a day, 7 days a week. After Pacific Hospice Care admission,
there is no paperwork, no bills, no claims, and no financial surprises.
